Amazon One Medical has expanded its cash-pay, on-demand GLP-1 weight-loss care to 48 states and Washington, D.C., adding video care alongside direct-message care where permitted. The service starts at $39 for direct-message care and $59 for video care, while medication costs are separate and prior authorization support is not included.

The expansion, reflected in a September 15, 2026 update to an announcement originally published April 21, gives patients two routes to weight-management care. One Medical offers insurance-based visits integrated with primary care in regions where it has offices, while the on-demand service provides standalone evaluation and prescribing for patients who prefer to pay out of pocket. One Medical membership is not required for the insurance-based program, and the company says it accepts most major insurance plans for scheduled in-person and video visits, although copays and deductibles may apply.

The primary-care GLP-1 Management Program includes pre-visit screening, consultations, follow-up visits, monitoring, standardized documentation and safety protocols. One Medical says the model allows clinicians to assess weight loss alongside cardiovascular health, diabetes, metabolic conditions and other aspects of a patient’s care. Clinicians can send prescriptions to the pharmacy selected by the patient.

The on-demand option is more narrowly focused on weight-loss evaluation and prescribing. It does not integrate treatment into a broader primary-care relationship or provide ongoing monitoring across medical conditions. Amazon One Medical describes the service as a transparent, pay-as-you-go option that does not require insurance.

  1. Patients complete a health screening and submit their most recent laboratory results for review.
  2. A licensed clinician evaluates the patient by direct message where available or by video.
  3. If treatment is clinically appropriate, the clinician can issue a prescription during the interaction.
  4. Patients may contact the provider with questions or concerns within 14 days of the initial visit.
  5. A new visit is required when patients need a dosage increase or are running low on medication.

Amazon Pharmacy lists upfront prices for GLP-1 medications and delivers to all 50 states. According to Amazon, insurance pricing for the Wegovy and Foundayo oral medications starts at $25 per month, while cash-pay pricing starts at $149 per month. Cash-pay prices for Wegovy injectables, the Zepbound auto-injector and KwikPen start at $199 per month. Same-Day Delivery currently reaches nearly 3,100 cities and towns, with Amazon planning to expand that coverage to nearly 4,500 by the end of 2026.

Patients using One Medical’s insurance-based program can compare insurance and cash-pay prices through Amazon Pharmacy. The company says eligible manufacturer coupons are applied automatically at checkout and that customers have saved more than $400 million through its automatic coupon program. Medication pickup is also available at select Amazon Kiosks within One Medical offices.
